- A list of current prescription or over-the-counter medications you are taking, including dose and frequency
- Pertinent information about your medical and surgical history
- Any pertinent x-rays or records you may have
Payment and Insurance
Evergreen Urology accepts cash, check, Visa, MasterCard, and most insurances. It is in your best interest to check your insurance providers list of participating physicians to be sure your plan allows you to be seen at our office.
Our billing office will assist with patient insurance forms. It is important that you read your policy in advance to be fully aware of the benefits available, and any limitations or restrictions that may apply to service you seek. Please remember that medical coverage is solely between you and your provider.

Fees
Fees for services at Evergreen Urology are based on the service or procedure provided. These charges are standard in our geographical area for comparable services. We encourage you to inquire in advance as to charges for any procedure. You will be provided with an accurate estimate. We request that payment for routine office visits and related charges be paid at the time services are rendered. This helps us to control billing costs and enables us to provide a more cost-effective service.
